How to Fill AADHAAR Form?

Instructions on how to fill AADHAAR application enrolment / correction / update form:

  • Fill all the required details in the form, do not leave any mandatory information empty.
  • Do not overwrite in the form; in case of mistakes discard and use a new form.
  • Fill the form carefully in capital letters only, using only standard fonts (avoid stylized writing) for easy readability and to minimize chances of any human mistakes.
  • Fill the AADHAAR application enrolment / correction / update form using black or blue ball point pen only, do not use ink pen or pencil to fill the form.
  • Date values must be filled properly in DD/MM/YYYY format.
  • Put a tick mark only in the boxes where options are to be selected as the answer, leave the other option(s) blank.
  • Leave the boxes empty where the requested detail is non relevant; do not write NA or N/A or NOT APPLICABLE.
  • Write clearly in the centre of the box without touching the box boundaries.
  • While filling the field, leave one box blank between each complete words.

Instructions for filling details in AADHAAR application enrolment / correction / update form:

  • Field 2 - UID (In case of Update): For AADHAAR correction / update, fill the 12 digits unique identification AADHAAR number (UID number) assigned to the applicant. Also, for AADHAAR correction / update, fill the Name and only the field(s) which needs update / correction.
  • Field 3 - Full Name: Write full name without salutations / titles. Ensure the name matches with the name that is written on the Proof of Identity document which the applicant is going to submit. Minor contrast in the name spelling is permitted as long as it does not differ completely from the name in the Proof of Identity document.
  • Field 5 - Age / Date of Birth: If the exact date of birth is not known, approximate age in years can be filled in the Age field. Else, write the exact date of birth in DD/MM/YYYY format in the Date of Birth field. If the applicant is going to submit a valid Proof of Date of Birth document, tick mark in the Verified box, else tick mark in the Declared box if no Proof of Date of Birth document is available.
  • Field 6 - Address: Write full address and ensure it matches with the address that is written on the Proof of Address document which the applicant is going to submit. Minor corrections / enhancements are permitted to make the address complete as long as it does not differ completely from the base address mentioned in the Proof of Address document. To include parent / spouse / guardian name as part of the address, tick mark the C/o option and enter the name of that person in the space provided.
  • Field 7 - Relationship: For children below 5 years of age, father / mother / guardian's name and their AADHAAR or EID number must be filled. For applicants who do not have a valid Proof of Identity document, name and AADHAAR or EID number of the Head of Family's must be filled. Relationship details are optional for other applicants and can leave it blank.
  • Field 8 - Documents: If required supporting documents are available, write the name of the Proof of Identity and Proof of Address documents. If supporting document for Proof of Date of Birth is available, write the name of the Proof of Date of Birth document. If required supporting documents are not available and applicant is applying through the Head of Family's introduction, write the name of the Proof of Relationship document.   • Field 9 - Introducer / Head of Family: Write the Introducer's AADHAAR number if the applicant is applying through the Introducer's introduction. Write the Head of Family's AADHAAR or EID number if the applicant is applying through the Head of Family's introduction. Please contact AADHAAR enrolment registration centre for further inquiries.
